Please be advised that ALL examination applications MUST be received by Friday May 12th, 2006 to be eligible to test on Sunday May 21st, 2006. The Jodo Kyu/Dan application and fees MUST be submitted to:

Please note, you must submit two separate cheques, one for the test(non-refundable) and one for the certificate(refunded if the test is not passed). Do not combine the two amounts into one cheque
